Requests to Access Information (Records)
There is no fee required to make a formal FOIP request to access your own personal information. Requests for General Information must be accompanied by an initial $25 fee. The request will be processed once the fee has been received.
NOTE: Please do not send cash through the postal system. Electronic and credit card payments are available and preferred; please inquire for instructions. Money orders or cheques must be made payable to: Grant MacEwan University. Please include a file number if applicable.
Request to Correct Your Own Personal Information
There is no fee required to request a correction of your own personal information.
Proposal to Access Personal Information for Research or Statistical Purposes
The researcher is responsible for paying any fees incurred by the University to search for, copy, and or provide the records.
Agreement to Access Personal Information for Research or Statistical Purposes
If the FOIP Head accepts the proposal to access personal information for research or statistical purposes, the applicant must complete and submit this agreement for signature by the FOIP Head.
Request for Review by Office of Information and Privacy Commissioner of Alberta (OIPC)
In the event an applicant disagrees with the decision by the Information and Privacy Office relating to a formal FOIP request. A request for review can be submitted directly to the OPIC.
Law Enforcement Disclosure - Generic
Use this form when law enforcement officials need to request MacEwan information, except for Edmonton Police Service (EPS) which has its own law enforcement access to information form.
Privacy and Information Security Breach Incident Report Form
A Privacy Breach occurs when there is loss, unauthorized access to, or disclosure of, personal information in physical or electronic form. Please note that to investigate an incident properly, we require a completed form with the details of your name, person(s), department, or faculty involved. You will be contacted when the Information and Privacy Office has reviewed the report.
